Our Cancellation Policy

At Form Health, we deeply value time—yours, and ours. Running an efficient practice enables us to care for more people and devote energy to research and education in the field. Our cancellation policy is designed to support this approach while respecting the commitment of both clients and physiotherapists.

Here’s how it works:

1. We prefer that you attend your appointment at the scheduled time.
   It helps us run smoothly and keeps care consistent.

2. No charge for cancellations made with more than 24 hours’ notice.
   Ideally, a few days' notice is appreciated so we can offer your spot to someone else.

3. Cancellations or rescheduling with less than 24 hours’ notice may incur a full fee.
   This fee may be charged to you or your insurer.
